Instructions
This form will send your request as an e-mail message to psclib1@pscvax.wvnet.edu. If you do not receive an e-mail confirmation from the ILL Office within 3 business days, please contact the ILL Office at psclib1@pscvax.wvnet.edu or 304-788-6903.
Please fill out the request form as completely as you can. Incomplete forms may cause delays in obtaining materials or notifying you that your item is ready to be picked up.
You must fill in all asterisked items. Requests with blank asterisked fields will be returned to the requestor.
Submitting a Request
You may submit a total of 5-10 book and photocopy ILL requests at one time. To do so, simply click on the submit button.
Privacy Statement
The ILL librarian and library staff respect your privacy and abide by the ALA standards on patron privacy rights. However, this form is not encrypted. This means that the Library cannot guarantee your privacy if you submit an electronic ILL request. If you wish to protect your privacy, you may fill out and print the form, and then take, fax, or mail it to the Library.

This area will explain what information is required and how to fill out the form properly.
Patron Information
- *Name:
- Please use the name on your library card.
- *PSC Library bar code:
- This is the bar code number on the back of your ID/library card. It should begin with P. For example, P97865. If you do not have a bar code, please come to the Library and get one before submitting a request. If you have lost your bar code, you must come in and replace it. There is a $5 replacement fee.
- *Patron status:
- Please indicate whether you are a student, PSC faculty/staff member, or community patron. This is used for statistics and doesn't affect the order in which your request is processed.
- Address:
- Please use your campus mailing address. On-campus students: Please indicate your PSC box number.
- *Phone:
- Please use your campus phone number.
- Fax:
- Please use a fax number that is available most of the time.
- *E-mail:
- This is required for all PSC faculty, staff, and students. To speed up the notification process, all PSC patrons will be notified by e-mail. Community patrons do not have to have e-mail.
- *Date needed by:
- This is the last possible date for which you can use the book. On this date, the OCLC ILL system terminates the request and returns it to the Library as "unfilled." It is also used by the Library to cancel requests.
Do not use ASAP. If you do not give a date, a need by date of 28 days is assumed.
If this is a rush request, type RUSH in the Comments box and indicate how much you are willing to pay for it in the "Maximum willing to pay:" field.

- If you cannot pay for the item, choose $0.
The ILL librarian always requests from free lenders first. If you must have the item, and there are no libraries willing to lend the book for free, how much are you willing to pay for the item? Lending fees range from postage to $5 to $10 to $20 per item.
Book Information
- Author:
- Please give the author's full name: Last_name, First_name.
- Title:
- Please give the complete title of the book, including any subtitles.
- Edition:
- The most recent edition will be requested unless you need a specific edition.
- Place of publication:
- Please give the city, state, and/or country in which the book was published.
- Publisher:
- Please give the name of the publishing company.
- Year of publication:
- The most recent year will be requested unless you give a specific year.
Most libraries will not lend new books for at least six months.
- ISBN (if known):
- International Standard Book Number. Example: ISBN 0-201-68862-X. It is the fastest and easiest way to locate the correct book.
- Comments:
- This space is for any special comments you need to make to the ILL librarian.
- Preferred Notification Method:
- How do you want the ILL librarian to notify you about the status of your request and its arrival? The fastest and most effective method is generally e-mail, but you must remember to check it daily.
- Citation Source:
- Where did you find the citation for this item? If there is a problem locating the correct edition or choosing similar but different items, the ILL librarian can refer back to the citation source. Otherwise, the request will be delayed as she attempts to contact you.
